1. Physical Clearance
Every athlete at Fremont High School is required to have a physical completed by a doctor authorizing they are physical able to play.
FUHSD provides two documents for athletic physicals
The first document is for parents to provide information and acknowledgement of the requirements of a doctor’s physical.
The second document is for the doctor to complete giving clearance to participate with date and signature.
Both forms can be accessed below.

Does my doctor have to use the district form?
No, your doctor does not have to use the district form but we are required to see a statement of physical clearance to play sports, a doctors signature (digital is okay), and a date of exam (exams are required annually).
Can a Nurse Practitioner complete my physical?
No, by law your physical must be completed by a MD, DO, or PA.
Where can I get my physical?
Most insurance providers offer an annual physical for free, we suggest you plan to get a physical every summer to ensure you are eligible for athletics all school year. You can also get athletic physicals through Wellness Mart but please be sure that your physical is signed by a MD, DO, or PA!!! Wellness Mart has had some NP signed physicals come through that are not valid.

2. Digital registration
